Please remember to include the following:

• a $300 application fee
• all necessary assessments, psychoeducational and neuropsychological evaluations (within the last 3 years)
• a letter of recommendation from a previous school
• a one-page parent narrative, describing what accomodations/modifications your child needs for academic and social success. (please be specific)
• recent photograph of applicant
