Required Forms for all students
The forms linked below are required for all Stanbridge families to complete upon enrollment.
Form data is gathered securely online via Google Forms using ReCAPTCHA, and printable versions of required forms will be sent home to families for final review and signature authorization.
As you cannot save progress when filling-out these forms online, a list of required documentation you will need is listed below for completing each set of forms. If you have any questions regarding these forms or data, please contact Linda Bargas in the main office.

EMERGENCY CONTACT FORM
• Basic demographic background of your child. • Your child's cell phone number, if applicable. • Street addresses, phone numbers, and email addresses for your family. • Required: Street addresses, phone numbers, and email addresses for two emergency contacts if school staff is unable to reach you. |
STUDENT MEDICAL INFORMATION
• Allergies your child has. • EpiPen requirement and expiry date, if necessary. • Date of student's last tetanus shot. • Your child's Covid vaccination status (provide copy of proof, if applicable). • Names, dosage amounts, and distribution instructions for any authorized meds. • Name and ID/Policy numbers of health & dental insurance providers. • Name and contact info for child's primary physician and dentist. |
WAIVERS & RELEASE FORMS
• Contact information for professionals your child may see (doctors, specialists, occupational therapists, and/or tutors). • Release forms and waivers, including Driver Authorization form if you will be transporting Stanbridge students in your personal vehicle for an off-campus, school-related event. NOTE: Technology Acceptable Use Policy and the Parent-Student Handbook Agreement both require student acknowledgment. |

FIELD TRIP PERMISSION FORM
• Required form for students traveling off-campus for a school-related field trip, activity, or athletic event. |
CALTRAIN RIDER PERMISSION
• Required form if your student will be traveling to/from campus via Caltrain in the mornings or afternoons. |
