Required Forms for all students
The forms linked below are required for all Stanbridge families to complete upon enrollment each year.
Form data is gathered securely online via Google Forms using ReCAPTCHA. Printable versions of the information you submit on required forms will be sent home to families for final review and signature authorization.
You will be unable to save progress when completing these online forms, therefore lists of the required documentation you will need to begin are included below. If you have any questions regarding these forms or data, please contact Linda Bargas.

EMERGENCY CONTACT
• Basic demographic background of your child. • Your child's cell phone number, if applicable. • Street addresses, phone numbers, and email addresses for your family. • REQUIRED: Street addresses, phone numbers, and email addresses for at least one additional emergency contact person if school staff is unable to reach a Parent/Guardian. |
MEDICAL INFO
• Allergies your child has. • EpiPen requirement and expiry date, if necessary. (Supply EpiPen on first day.) • Date of student's last tetanus shot. • Date of student's last Covid vaccination, if applicable. • Names, dosage amounts, and instructions for any authorized meds. • Name and ID/Policy numbers of health & dental insurance providers. • Contact info for primary physician & dentist. NOTE: Please also see the current state schedule for childhood immunizations; provide a copy of current record to the Front Office:

WAIVERS & RELEASES
• Contact information for your child's professionals (doctors, specialists, occupational therapists, and/or tutors). • Release forms and waivers. REQUIRED: Both the Parent-Student Handbook Agreement & Technology Acceptable Use Policy require student acknowledgment. NOTE: Parent eMails will be securely shared with a professional photographer for portraits. Your address will not be shared further or used for solicitation. |
FIELD TRIP PERMISSION
NOTE: Annual permission of off-campus travel for any school-related daytime or evening activities. Includes standard expectations for general participation and travel by Stanbridge shuttle or public transportation. Waivers for overnight experientials will be sent separately, prior to specific trips. |
Additional Forms as needed
CALTRAIN RIDER PERMISSION
• Required permission for students traveling to/from campus via Caltrain in the mornings or afternoons. • Additional permission for solo student travel to station in the afternoon. |
PARENT DRIVER AUTHORIZATION
• Required form if you will be transporting Stanbridge students in your personal vehicle for an off-campus, school-related event |
SPA VOLUNTEER SURVEY
Your participation directly contributes to the continued successes that benefit our students in every grade. Let us know how you want to become involved this year! |
ATHLETICS PERMISSION
• Required completion if your child has made the decision to participate in Stanbridge Academy Athletics, part of the co-ed Bay Area Small School Athletic League (BASSAL). |
